When the slow song comes on... go up to and say do you want to dance. If she says yes take her by the hand and put your hands on her hips ( not in a freaky way) and she may put her hands on your shoulders and lay her head on your shoulder, dont act nervous just be cool about it and act confident.

Just go up to her and kinda make a joke about it like ';hey you wanna dance.'; Sound playful don't make it sound like you shy. About the dance you could hold her right hand and put ur left hand around her back like 2-stepping.
I'm assuming you're already at the dance when the occasion occurs. Verbally asking never seemed to come up for me. I just went up to the girl when the slower music began and asked her if she cared to dance. The music speaks for itself. She'll know you are going to ';slow dance';, so, if she stands up and comes toward you, she's saying yes. As ';Dancer92'; said, put your hand on her hip, ';not in a freaky way'; to use ';Dancer92'; 's words and put your other hand up to hold her's, and then, dance. It all comes about much easier than you think, since, as I said, the music speaks for itself. She'll know you want to slow dance when she hears the music, so, just stay calm, walk up to her, maybe hold a hand out to her, and ask her if she'd dance with you.
