Provide us with more information about your situation...are you guys friends? How do you know her? What is your relationship like? Do you hang out? Have mutual friends, etc?

I wouldn't specify that the dance would be a ';date'; though...keep your question open-ended, you could go as friends and still have it turn into something. It'll put less pressure on you and her...

And without knowing the details, I would always advise going for it. You never make a fool of yourself by taking a risk and asking someone you like out. Even if (worst case scenario) you get rejected, you'll get over it, and it's always good to know you can get over rejection. Good luck!
Next time you see her alone, but if you can not completely alone ( a.k.a if she is alone but there our other people having conversations in the hall or something.) Walk up to her with a calm, proud stance, and just ask her. if you need more help use this hi there.

'; Hey There ( Insert Name) did you find a date to the dance yet?';

1st. '; Uh Yea I'm going with ( Insert Name)';

RE: '; Hmm... well thats cool, i need to go to my locker. ( In anticipation do not go to your locker before.';

2. '; No....Why?';

RE: Well I was wondering if you wanted to go with me.';

If she says no play it cool and keep a conversation flowing to show it doesn't bother you. If she says yes send me 20$ lol.
Ask her her plans for the dance. If she isn't spoken for, just ask if she would like to accompany you.
